Release of Liability, Waiver of Claims, Assumption of Risk, and Indemnity Agreement for participation in IGN Academy basketball leagues, evaluations, practices, and related activities (the “Activities”), Conway, Arkansas. Please read carefully — by checking the box you are giving up substantial legal rights.
1. Voluntary Participation & Assumption of Risk
I understand that basketball and related athletic activities involve inherent risks of injury — including but not limited to sprains, fractures, concussions and other head injuries, contact with players, floors, walls, equipment, and, in rare cases, serious injury, permanent disability, or death. I understand these risks cannot be eliminated regardless of the care taken. Knowing these risks, I voluntarily choose for myself and/or my child (the “Participant”) to take part in the Activities and assume full responsibility for all such risks.
2. Health & Fitness
I certify that the Participant is physically fit and has no medical condition that would prevent safe participation. I understand IGN Academy does not provide health or accident insurance for participants and that I am responsible for the Participant’s own insurance coverage and all medical costs.
3. Release & Waiver of Claims
In consideration of being permitted to participate, I, on behalf of myself, the Participant, and our heirs, assigns, and next of kin, release, waive, and discharge IGN Academy, its owners, coaches, staff, volunteers, referees, sponsors, and the owners of any facility used (the “Released Parties”) from any and all claims, demands, or causes of action arising out of injury, loss, or damage connected with the Activities — including claims based on the ordinary negligence of the Released Parties — to the fullest extent allowed by Arkansas law.
4. Indemnification
I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost (including attorney’s fees) they may incur arising from the Participant’s involvement in the Activities.
5. Medical Authorization
In the event of injury, I authorize IGN Academy staff to administer basic first aid and to arrange emergency medical treatment and transport if I cannot be reached. I accept financial responsibility for any medical care provided.
6. Code of Conduct
I agree that the Participant, and I as a spectator, will follow all league rules and the reasonable direction of coaches and officials, and will treat players, officials, staff, and other families with respect. IGN Academy may remove any participant or spectator for conduct it deems unsafe or unsportsmanlike, without refund.
7. Photo & Media Release
I grant IGN Academy permission to use photographs and video of the Participant taken during the Activities for promotional and social-media purposes, without compensation. If you do not consent to this, email IGN Academy in writing before your first session.
8. Personal Property
IGN Academy is not responsible for lost, stolen, or damaged personal property.
9. Refunds
Registration fees hold your spot and cover league costs committed in advance. No refunds once your spot is held.
10. Governing Law & Severability
This agreement is governed by the laws of the State of Arkansas. If any part is found unenforceable, the remainder stays in full effect. I confirm I am the Participant, or the parent/legal guardian of a minor Participant, and that I am at least 18 years old and have authority to sign on the Participant’s behalf.
By checking the waiver box on the registration form, I acknowledge that I have read and understood this entire agreement and agree to be bound by it.
